Position Description

ROLE:  

The Post-Acute Care Advanced Practice Clinician (Nurse Practitioner/Physician Assistant) employed by MedOne is responsible for managing health problems and coordinating health care for the post-acute care patient. The APC will collaborate with a physician (s) to provide full scope of patient care in accordance with State, Federal and institutional rules and regulations.  The Nurse Practitioner practices in collaboration with one or more physicians on his/her Standard Care Arrangement.  The Physician Assistant practices under the supervision of one or more physicians on his/her Supervisory Agreement.  

 

QUALIFICATIONS: 

 

NURSE PRACTITIONER 

PHYSICIAN ASSISTANT 

Current license in the state of Ohio as a registered nurse and nurse practitioner 

Graduate of an accredited physician assistant program 

Graduate of approved Nurse Practitioner program 

Current national certification by National Commission on Certification of Physician Assistants (NCCPA) 

 

Current state license and registration 

Certified in BLS  endorsed by the American Heart Association 

Certified in BLS endorsed by the American Heart Association 

Signed Standard Care Agreement with collaborating physicians 

Signed supervisory agreement (s)  on file with the state medical board 

 

Have obtained initial provisional certificate or certificate to prescribe 

Have a valid DEA registration 

Have a valid DEA registration 

Have access to Ohio Automated Rx Reporting (OAARS) 

Have access to Ohio Automated Rx Reporting (OAARS) 

Eligible for Post-Acute Care privileges

ORGANIZATIONAL RESPONSIBILITIES: 

 

Demonstrates MedOne values through positive and effective interactions with patients, families and medical team.  Promotes a positive team attitude via mutual respect for co-workers, developing positive interpersonal relationships and assisting other team members as needed.  

 

RESPONSIBILITIES: 

  • Evaluates new and existing patients by performing a history and physical and interpreting laboratory and diagnostic studies for acute and chronically ill patients on the MedOne post-acute care service. Recognizes deviations from normal physical exam and laboratory/diagnostic studies.   
  • Works in collaboration (nurse practitioners) or under the supervision (physician assistants) with a physician in developing treatment plans for health problems and follow-up.  
  • Orders and interprets diagnostic and laboratory tests related to the patient’s needs. 
  • Prescribes appropriate pharmacologic medications in accordance with prescriptive authority regulations.   Prescribes appropriate non-pharmacologic treatment modalities.  
  • Monitors the effectiveness of interventions and communicates to the collaborating/supervising physician as needed.  
  • Collaborates with multidisciplinary team members by making appropriate referrals and consultations.  
  • Facilitates the patient’s transition within and between health care settings such as admitting, transferring and discharging patients.  
  • Communicates treatment plan between patient, family and the medical team. 
  • Maintains patient and family confidentiality, privacy and patient rights. 
  • Documents in the patient record as appropriate according to MedOne and institution guidelines.  
  • Adheres to credentialing and privileging process in order to maintain clinical privileges.  
  • Bills or logs encounters as directed.  
  • Periodically has on-call responsibilities during designated after hours 
  • Acts as a role model and resource to other advance practice clinicians and nursing staff.  

 

STAFF DEVELOPMENT 

 

  • Pursues knowledge in current standards of care (evaluation and treatment of disease processes) by participating in continuing education opportunities and professional organizations. 
  • Pursues advancement in professional development through direct care, research, education, leadership or other scholarly activities.
