Director of the Ohio Long-Term Care Research Project,
Senior Research Scholar, Scripps Gerontology Center
Dr. Robert Applebaum is the Director of the Ohio Long-Term Care Project at the Scripps Gerontology Center, Miami University, Oxford, Ohio. He holds degrees from Ohio University (BA), the Ohio State University (MSW), and the University of Wisconsin-Madison (Ph.D.). He has been involved in the development and evaluation of long-term care programs across the United States for more than thirty-five years. He has worked with more than 25 states on innovations in long-term care service delivery, and completed a series of state and national studies on long-term services and supports. Dr. Applebaum has been a frequent speaker at national and state conferences on long-term care. He has authored more than 100 articles and monographs, and four books on long-term care and has testified at state and federal legislative hearings more than twenty times. He has been the Principal Investigator on more than 60 studies and received more than $12 million in external grants. Dr. Applebaum has been active in serving the Gerontological Society of America and the American Society on Aging. He has chaired the Social Research Planning and Practice Section of the Gerontological Society and has been a member of the Board of Directors of the American Society on Aging and Chair of the Editorial Board for the Society’s journal, Generations.

She earned her Master's in Nursing in 1999, marking the beginning of a passionate and impactful career in the field. She is currently the State Clinical Director, South Regional Clinical Lead and is involved heavily with Corporate Relations/Business Development for Eventus WholeHealth. Professional Background: Kimberley's expertise lies in the nuanced and essential realm of long-term care. She has consistently demonstrated a commitment to providing compassionate, patient-centered care that goes beyond routine medical attention. With a keen understanding of the unique needs of the aging population, Kimberley has become a trusted advocate for her patients, ensuring their comfort and well-being are prioritized. Educational Achievements: Graduating with distinction, Kimberley completed her Master's in Nursing with a focus on advanced practice. This educational foundation has empowered her with the knowledge and skills necessary to navigate the complex landscape of long-term care, blending clinical acumen with a deep understanding of the human aspect of healthcare. Terrence A. O’Malley, M.D. is a retired geriatrician with a forty-seven-year career at Massachusetts General Hospital where he was part of the first primary care internal medicine class and served on the teaching service for twenty years. He was medical director of the Chelsea Primary Care practice and of several local nursing facilities. For fifteen years he served as the Medical Director, Non-Acute Care Services at Partners HealthCare in Boston where he oversaw Partners’ LTACs, IRFs, SNFs and Hospice/Home Health Agency. In this role he initiated and directed a network-wide transitions of care program that improved the completeness and timeliness of information shared during a care transition from Partners’ hospitals to other sites of care. He was co-PI on an ONC Challenge grant that defined the content of post-acute care transitions and resulted in a CCDA R2 standard and participated in the S&I Framework on Post-acute Care Transitions, Longitudinal Coordination of Care, and the Electronic Long-Term Services and Supports Work Group (eLTSS). He served several terms on the ONC HITAC (Health Information Technology Advisory Committee) and co-chaired the USCDI Taskforce while participating in Gravity, 360X, and the LTPAC HIT Collaborative, and served on numerous technical expert panels. He currently co-chairs the PACIO Project Transitions of Care Workgroup which is developing role-based content standards for post-acute care transitions. He also co-chairs the Moving Forward Coalition HIT Committee which is developing standards for an individual's goals, preferences, and priorities and refining an HIT maturity model for SNFs to help guide HIT adoption. He is a board member of the Long-Term Quality Alliance (LTQA), serves on the NCQA Geriatric Measurement Advisory Panel (GMAP). and is a Corresponding Member of the Faculty of Medicine at Harvard University.

Graduated from University of Toledo College of Pharmacy after working in Pharmacy operations for 20 years. Alan is a past president of Ohio ASCP, The Treasurer of Ohio Medical Directors Association, was an alternate for the Governor’s Task Force on Long Term Care, and a member of the ASCP Choosing Wisely Taskforce. Alan believes that nursing home practice is a team sport and consultant pharmacists should be working directly with Nurse Practitioners whenever possible.

Pursued a career in pharmacy and graduated from University of Toledo College of Pharmacy after working in a hometown nursing home while in high school and through college. Sarah was able to stay in long-term care working as a Consultant Pharmacist after graduation. She is from northeast Ohio and currently the Lead Consultant Pharmacist for the Midwest Region at Remedi SeniorCare.
Ohio CMO for Eventus Whole Health
Dr. Dirk Juschka is a highly experienced physician with over 25 years dedicated to long-term care. After earning his M.D. in 1981, he initially practiced in hospitals, offices, and nursing homes before focusing exclusively on long-term care. He became a Certified Medical Director (CMD) in 2012. Dr. Dirk has served as medical director at more than 30 skilled nursing facilities and actively participates in approximately 30 QAPI meetings each month. His first collaboration with nurse practitioners began in 1991 and his practice evolved into a priority for nurse practitioners in skilled nursing care. Recently, he became the Ohio Chief Medical Officer for Eventus. The Eventus team provides an NP forward approach to a wide range of medical services for skilled nursing and assisted living facilities across seven states.

Jeana Singleton has a national healthcare law practice focusing on healthcare operational matters, including state and federal regulatory compliance, healthcare transactions, healthcare policy, public health initiatives, corporate law, and business-legal strategy. She has a special interest in matters involving advanced practice providers. Since 2007, Jeana has served as general counsel for the Ohio Association of Advanced Practice Nurses. Throughout her career, Jeana has had the privilege to assist advanced practice providers start practices, expand practices, navigate regulatory issues, pursue policy changes to reduce practice barriers, and tackle licensure issues. Jeana is licensed in Ohio, Michigan and Tennessee.
